Immunizations
To comply with Arizona State Law (A.R.S. §§ 36-671 and 15-871), students must have a current immunization record on file with the school in order to attend school, unless an appropriate exemption has been submitted.
Arizona law requires that a student's immunization history be provided at the time of enrollment. Parents and guardians are responsible for keeping the school health office informed of any additional immunizations their child receives throughout the school year. Students must remain current on all required immunizations.
Additional Immunizations for Students Ages 11 and Older
Students who are 11 years of age or older are required to have:
- One dose of the Tdap (tetanus, diphtheria, and pertussis) vaccine
- One dose of the meningococcal vaccine
Immunization Exemptions
Arizona law allows parents or guardians to request an immunization exemption for:
- Medical reasons
- Religious beliefs
- Personal beliefs
The appropriate Immunization Exemption Form must be completed and signed by a parent or guardian.
